Season 26 kicks off with Nkele and Tshililo saying a heart-wrenching farewell to Reneilwe at her cremation. Meanwhile, Dr. Ximba (Leleti Khumalo) makes a powerful entrance at MMC, running into Imani. Ndiwavho is distressed about relocating to Venda, and Teboho is compelled to eliminate a prison gang leader.

In a desperate move, Teboho frames her cellmate to protect herself. Imani faces anxiety as she tries to impress Dr. Ximba. Bubbles brings the church women to offer their condolences to Nkele, but the moment is disrupted by a bank demand letter. Tshililo attempts to alert his associate as KK’s enforcer closes in on him.

Tshililo’s associate reveals the elimination of KK’s person. Ndemedzo (Wiseman Zitha) pretends to be a devoted admirer of Dr. Ximba’s work while concealing his true intentions. Teboho surprises everyone by attending a Hayani Holdings meeting as Nkele’s representative. Tshililo and Imani’s intimate dinner is disrupted by the police. A distressed Susan decides to end her relationship with Avhatakali but ends up being intimate with him at Dr. Ximba’s house, only to be shot by a concealed woman.

Susan is rushed to the hospital in critical condition. Avhatakali gets a frantic call from the hospital requesting Dr. Ximba. Nkele is kidnapped by two attackers. The hospital head implores Dr. Ximba to perform a life-saving procedure on Susan. Will she step up to save her husband's mistress?
